Preheat oven to 350 F.
Stir together oats and milk and let sit until milk is mostly absorbed.
Thinly slice 1/4 of the onion and set aside. Finely chop remaining onion.
Add turkey, chopped onion, eggs, Worcestershire, ketchup, salt, and pepper into oat mixture. Mix until well combined.
Transfer mixture to a 9x13 inch baking dish and shape into a loaf about 5 inches wide and 2.5 inches high. Lightly top with ketchup and sliced onions.
Bake for 1 hour. Remove from oven and let rest for 15 minutes before slicing.
