Preheat oven to 400. Thoroughly mix all ingredients by hand, ensuring even distribution. Spread meat mixture on bottom of 9x13 baking dish, or multiple smaller ones. Bake for 40-55 minutes. (Each oven seems to cook this differently, when in doubt, check it before you serve)
