Mix pudding and milk well. Fold in Cool whip. Butter 9 x 13 pan. Place layer of graham crackers in bottom. Spread half pudding over crackers. Top with second layer of crackers. Spread remaining pudding over top. Add 3rd layer of crackers.
Combine milk, cocoa, sugar and salt in sauce pan and boil 1 minute. Remove from heat add butter and vanilla. Mix well - Cool Spread on cake. Refrigerate.
EASIER TOPPING. Purchase can of milk chocolate frosting. Warm slightly in micro-wave. Pour over cake.
