Stir together sugar, cornstarch, and salt in a saucepan. Mix milk, heavy whipping cream (optional) and egg yolks. Gradually add milk and egg combination to sugar combination. Cook over medium heat until boiling and thick continuously stirring. When boiling, continue to stir for one minute. Remove from heat and add vanilla and chocolate. Immediately pour into pie shell.
