To make sauce, stir first 7 ingredients together (soup - bay leaf) being careful not to break the bay leaf.
In the crockpot, place the onion, followed by the chicken and then the mushrooms. Pour sauce over all and cook on low 4 to 5 hours until the flavors have blended and the sauce is thick and bubbly.
Remove bay leaf and serve over rice or noodles.
